APPLICATION FOR UNABRIDGED DEATH CERTIFICATE

MUST BE SUBMITTED BY POST. 
 
Any person may apply for a copy of a Death Certificate provided that the deceased person’s death has been registered with the South African Home Affairs Department.
 
If a foreigner died in South Africa, he/she will be manually registered with South African Home Affairs and will be issued with handwritten death certificate.  
 
If a South Africa died in UK, he/she will be manually registered with South African Home Affairs and will be issued with a handwritten receipt.
 
The tracing of original records in the register held by the Department of Home Affairs in Pretoria is a time-consuming task and, as a result, the processing time of applications is likely to take approximately 12 months or even longer. It is advisable to submit copy of the handwritten death certificate/handwritten receipt if applying for the death certificate in RSA.
 
Details on the application must be supplied according to the registration at the time of death and not of names assumed in the family.
 
DEATH CERTIFICATE REQUIREMENTS
 
1. Form DHA/BI-132: Must be completed fully in BLOCK LETTER and BLACK INK and submit together with two copies of the following documents:
  • Copy of previous death certificate.  This will facilitate the tracing and processing of the record.
  • Copy of ID document/passport of the deceased 
2. A pre-paid, A4 size, self-addressed, Special Delivery envelope (which indicates 500grm paid for). 
3. Processing fees (Non-Refundable) £7.00. It must be paid in UK bank and in Sterling Pounds. It must be deposited electronically into the High Commission’s Account using the below information:

NP: Please be reminded that you will be required to submit printed proof of payment when submitting/posting the application.
Payment must be made three working days before the submission of your application.
 
N.B. NO Postal orders, Cheques or CASH payments will be accepted.
 
Please Note: To enable to facilitate the tracing of records and processing of the application, please submit fully completed applications including the Identity number and the death entry number if possible.
On receipt of the certificate, it will be posted in the pre-paid, self-addressed special delivery envelope provided at the time of submission of your application.
