APPLICATION FOR UNABRIDGED BIRTH CERTIFICATE

MUST BE SUBMITTED BY POST

Details on the application must be supplied according to the registration at birth and not of names assumed in the family. If the name/forename change has taken place in South Africa through the Department of Home Affairs, a copy of approved letter or of a new amended certificate is required.

REQUIREMENTS:

  • Form DHA-154 –Must be completed fully in BLOCK LETTER and IN BLACK INK.
  • Form DHA-24 and Form DHA-288 (for all those who do not have ID numbers on their birth certificates or those with very old ID book and for those born between the years 2000-2004.
    The bar-coded forms such as DHA-24, which cannot be downloaded, will be provided on the day of your appointment. You must book an appointment to come to the office only if you have to complete the form DHA-24 with your birth certificate application.

1. Copy of any previous birth certificate (will facilitate the tracing and processing of the record) x 2 copies

2. Copy of ID document/passport x 2 copies

3. Copy of parents’ ID documents and passports x 2 copies

4. A pre-paid A4 size self-addressed special deliver envelope (which indicates 500grm paid for).

  • Please note: If 1st class stamped envelope is provided, it will be posted at your own risk.

5. Processing fees (Non-Refundable) of £7.00. It must be paid in UK bank and in Sterling Pounds, it must be deposited electronically into the High Commission’s Account see the information below:

NP: Please be reminded that you will be required to post printed proof of payment when submitting/posting the application.

Please Note: To enable to facilitate the tracing of records and processing of the application, please submit fully completed applications including the Identity number or birth entry number.

The tracing of original records in the register held by the Department of Home Affairs in Pretoria is a time-consuming task, as a result processing of applications is likely to take approximately 12 months or longer.

On receipt of the certificate, it will be posted in the pre-paid, self-addressed envelope provided at the time of submission of your application at your own risk.
