  • BI-132 Application for Death Certificate form
  • Certified copies of South African document of the deceased
  • Fee – CAN $6.00 in money order or certified cheque payable to the “South African Consulate General” in Toronto or to the “South African High Commission” in Ottawa. No cash, please. No personal cheques, please.
  • Proof of ID of the applicant and relationship to the deceased required.
  • Please note that only next of kin will be provided this copy
