{"id":4136,"date":"2025-09-19T15:38:35","date_gmt":"2025-09-19T12:38:35","guid":{"rendered":"https:\/\/dirco.gov.za\/uk\/?page_id=4136"},"modified":"2025-10-08T11:01:50","modified_gmt":"2025-10-08T08:01:50","slug":"change-of-surname-under-18-years-old","status":"publish","type":"page","link":"https:\/\/dirco.gov.za\/uk\/change-of-surname-under-18-years-old\/","title":{"rendered":"CHANGE OF SURNAME (UNDER 18 YEARS OLD)"},"content":{"rendered":"\t\t<div data-elementor-type=\"wp-page\" data-elementor-id=\"4136\" class=\"elementor elementor-4136\" data-elementor-post-type=\"page\">\n\t\t\t\t<div class=\"aux-parallax-section elementor-element elementor-element-d05ee9a e-flex e-con-boxed e-con e-parent\" data-id=\"d05ee9a\" data-element_type=\"container\" data-e-type=\"container\">\n\t\t\t\t\t<div class=\"e-con-inner\">\n\t\t\t\t<div class=\"elementor-element elementor-element-d23db40 elementor-widget elementor-widget-heading\" data-id=\"d23db40\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<h2 class=\"elementor-heading-title elementor-size-default\">CHANGE OF SURNAME (UNDER 18 YEARS OLD)<\/h2>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t<div class=\"aux-parallax-section elementor-element elementor-element-bd32a9a e-flex e-con-boxed e-con e-parent\" data-id=\"bd32a9a\" data-element_type=\"container\" data-e-type=\"container\">\n\t\t\t\t\t<div class=\"e-con-inner\">\n\t\t\t\t<div class=\"elementor-element elementor-element-d21a068 elementor-widget elementor-widget-text-editor\" data-id=\"d21a068\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>This amendment is in terms of Section 26(2) of the Births and Deaths Registration Act, 1992.<\/p><p>Application requirements for <em>corrections<\/em>, <em>re-registration of a birth<\/em>, <em>insertion of the biological father&#8217;s particulars<\/em>, <em>change of forenames of an adult or child<\/em>, <em>change of the surname of a minor<\/em>, change of gender, alteration of the date of birth are different and the applicable guidelines should be consulted.<\/p><p><strong><u>Appointment Requirements<\/u><\/strong><strong>:<\/strong><\/p><ol><li><span style=\"color: #ff0000;\"><strong>Ensure to come with BLACK INK pen <\/strong><\/span><\/li><li><strong>Both the parents must attend the appointment if the person is below 18 years of age<\/strong><\/li><li><strong>The child who is 15 years old and above, must attend the appointment<\/strong><\/li><li><strong>Passport must be valid for at least 4 years before you submit the application for change of forenames.\u00a0 <\/strong><\/li><li><strong>All application forms must be fully completed but not signed, as they must be signed in the presence of a Home Affairs Official.<\/strong><\/li><li><span style=\"color: #ff0000;\"><strong>Ensure to come with all originals and 2 x copies of supporting documents, <\/strong><span style=\"color: #000000;\"><strong>the official will certify the copies.<\/strong><\/span><\/span><\/li><li><strong>All foreign documents MUST be officially translated into English language.<\/strong><\/li><li><strong>Printed proof of payment<\/strong><\/li><li><span style=\"color: #ff0000;\">A pre-paid self-addressed A4 envelope<\/span> (which indicates 500grm paid for).<ul><li>to post the outcome on receipt from the Department in South Africa.<\/li><\/ul><\/li><li><strong>The child can only apply for the new passport in the new surname once he\/she receives his\/hers unabridged birth certificate with the new surname or written confirmation from the Department stating that the change of surname has been approved. <\/strong><\/li><\/ol><p><strong>Prescribed forms and Documents <\/strong><\/p><p><strong>Forms which cannot be downloaded will be provided when you come to your appointment.<\/strong><\/p><p><strong>All forms must be fully completed in BLOCK LETTERS and BLACK INK<\/strong><\/p><ol><li><strong>Form <a href=\"https:\/\/dirco.gov.za\/uk\/wp-content\/uploads\/sites\/5\/2024\/06\/DHA-BI-193-CHANGE-OF-SURNAME-MINOR.pdf\">BI-193 <\/a><\/strong><\/li><li><strong>DHA-24LRB<\/strong> &#8212; Will be given on the day of the appointment. Must complete two similar forms. (1 x to be completed in a new surname &amp; 1 x old surname as it is on the SA birth certificate\/SA passport).<\/li><li><strong>Form DHA \u2013 1688 <\/strong>It will be given on the day of the appointment.<\/li><li><strong>FORM DHA-9<\/strong> &#8212; (if the child is 16 years old and above) It will be given on the day of the appointment.\\<\/li><li><a href=\"https:\/\/dirco.gov.za\/uk\/wp-content\/uploads\/sites\/5\/2025\/06\/DHA-529.pdf\"><strong>DHA\/BI 529<\/strong><\/a> to be completed in <strong>BLACK INK and BLOCK LETTERS. <\/strong>\u00a0<strong>(FULLY COMPLETED means all questions on the two (both sides) forms must be answered. Please do not answer \u201cN\/A\u201d, it is not an answer to any of the questions. <\/strong><ul><li><strong>Part A:<\/strong> questions 9, 10 and 11 are most important for the applicant. <strong><u>B.<\/u><\/strong> No.9 dates when you came to work\/live\/accompanied with a parent in UK for the 1<sup>st<\/sup> time.<strong><u> number 10(d) must also be answered, with circling a No or a Yes. \u201cYes\u201d, if you previously applied for a Retention and attach your 1<sup>st<\/sup> Retention copy.<\/u><\/strong><\/li><li><strong>Part B:<\/strong> The relevant status can be written next to the words Marital Status on the form (e.g., Never married\/ Married\/ Divorced\/Widowed). The details of the applicant\u2019s current or former spouse must then be completed on the DHA-529 whether deceased or divorced or widowed)<\/li><li><strong>Part C and D: <\/strong>Must both be completed in full, with the details of the parents of the applicant including their ID numbers and details of foreign nationality (if any)<\/li><li><strong>Part E<\/strong>: Do not sign the form as it must be signed in the presence of Home Affairs Official. \u00a0Ensure that valid telephone\/mobile number, email address (in block letters) and physical address is be provided.<\/li><\/ul><\/li><li><strong>Form <a href=\"https:\/\/dirco.gov.za\/uk\/wp-content\/uploads\/sites\/5\/2024\/04\/DHA-154.pdf\">DHA-154<\/a><\/strong> (to complete in new surname),<\/li><li><strong>Form <a href=\"https:\/\/dirco.gov.za\/uk\/wp-content\/uploads\/sites\/5\/2024\/06\/DHA-SVP-109-NAMABIAN-QUESTIONER.pdf\">SVP \u2013 109E<\/a><\/strong> &#8212; For citizens born in Namibian to be completed fully <strong>in BLACK INK and BLOCK LETTERS.<\/strong><\/li><\/ol><p><strong><u>DOCUMENTS of the child <\/u><\/strong><\/p><p><strong>You must come with the <\/strong><span style=\"color: #ff0000;\"><strong>Original and two copies <\/strong><\/span><strong>of each document, the official will certify the copies. \u00a0<\/strong><\/p><p><strong>All foreign documents MUST be officially translated into English language<\/strong><strong>.<\/strong><\/p><ul><li>Current passport<\/li><li>Vault copy of Unabridged Birth Certificate of the child where both the parents details appear.<\/li><li>Signed statement by both the parents confirming why they both wish to change child\u2019s surname.<\/li><li>Evidence of usage of the surname to be registered. This may be in the form of a statutory declaration by a South African citizen who is prepared to state that this is the surname currently being used.<\/li><li>Signed consent from surname holder with a certified copy his\/her passport and ID document.<\/li><li>Four (Six for the child who is 16 years old and above) very clear professionally taken colour photos of the full face of applicant, showing only head and shoulders measuring 35mm x 45mm. Without glasses and clear forehead (Not covered by hair)<\/li><li>If the child has dual nationality, please provide proof as to how and when the foreign nationality was acquire\/granted, such Naturalization \/Registration Certificate or foreign birth certificate issued by the Embassy\/ letter from the authorities stating the date you acquired the nationality. (Please note foreign passport is not a proof of foreign citizenship\/nationality as it does not indicate as to when(date) and how you acquired the citizenship).<\/li><li>g.
