{"id":4135,"date":"2025-09-19T13:00:29","date_gmt":"2025-09-19T10:00:29","guid":{"rendered":"https:\/\/dirco.gov.za\/uk\/?page_id=4135"},"modified":"2025-10-08T12:55:51","modified_gmt":"2025-10-08T09:55:51","slug":"change-of-forenames-for-minor","status":"publish","type":"page","link":"https:\/\/dirco.gov.za\/uk\/change-of-forenames-for-minor\/","title":{"rendered":"CHANGE OF FORENAMES FOR MINOR"},"content":{"rendered":"\t\t<div data-elementor-type=\"wp-page\" data-elementor-id=\"4135\" class=\"elementor elementor-4135\" data-elementor-post-type=\"page\">\n\t\t\t\t<div class=\"aux-parallax-section elementor-element elementor-element-4e41bf0 e-flex e-con-boxed e-con e-parent\" data-id=\"4e41bf0\" data-element_type=\"container\" data-e-type=\"container\">\n\t\t\t\t\t<div class=\"e-con-inner\">\n\t\t\t\t<div class=\"elementor-element elementor-element-c60af6e elementor-widget elementor-widget-heading\" data-id=\"c60af6e\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<h2 class=\"elementor-heading-title elementor-size-default\">CHANGE OF FORENAMES FOR MINOR (UNDER 18 YEARS OLD)<\/h2>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t<div class=\"aux-parallax-section elementor-element elementor-element-4a71779 e-flex e-con-boxed e-con e-parent\" data-id=\"4a71779\" data-element_type=\"container\" data-e-type=\"container\">\n\t\t\t\t\t<div class=\"e-con-inner\">\n\t\t\t\t<div class=\"elementor-element elementor-element-3713bf6 elementor-widget elementor-widget-text-editor\" data-id=\"3713bf6\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>This amendment is in terms of Section 26(2) of the Births and Deaths Registration Act, 1992.<\/p><p>Application requirements for <em>corrections<\/em>, <em>re-registration of a birth<\/em>, <em>insertion of the biological father&#8217;s particulars<\/em>, <em>change of forenames of an adult or child<\/em>, <em>change of the surname of a minor<\/em>, change of gender, alteration of the date of birth are different and the applicable guidelines should be consulted.<\/p><p><strong>To apply for the change of forenames for minor you must comply with the following<\/strong><strong> requirement:<\/strong><\/p><p><strong>Appointment Requirements:<\/strong><\/p><ol><li><span style=\"color: #ff0000;\"><strong>Ensure you bring a BLACK INK with pen <\/strong><\/span><\/li><li><strong>Both parents must attend the appointment if the applicantisbelow 18 years of age.<\/strong><\/li><li><strong>If the child is 15 years old and above, the child must attend the appointment<\/strong><\/li><li><strong>Passport must be valid for at least 4 years before you submit the application for change of forenames. \u00a0<\/strong><\/li><li><strong>All application forms must be fully completed but not signed, as they must be signed in the presence of a Home Affairs Official.<\/strong><\/li><li><span style=\"color: #ff0000;\"><strong>Ensure to come with all originals and 2 x copies of supporting documents, <\/strong><\/span><strong>the official will certify the copies.<\/strong><\/li><li><strong>Printed proof of payment<\/strong><\/li><\/ol><ol start=\"8\"><li><span style=\"color: #ff0000;\">A pre-paid self-addressed A4 envelope<\/span> (which indicates 500grm paid for). to post the outcome on receipt from the Department in South Africa.<\/li><\/ol><ol start=\"9\"><li><strong>The child can only apply for the new passport in the new forenames once he\/she receives his\/hers unabridged birth certificate with the new forenames or written confirmation from the Department stating that the change of forename\/s has been approved. <\/strong><\/li><\/ol><p><strong>Prescribed forms and Documents <\/strong><\/p><p><strong>Forms which cannot be downloaded will be provided when you come to your appointment.<\/strong><\/p><p><strong><u>Prescribed forms<\/u><\/strong><u>:<\/u><\/p><p><strong>All forms must be fully completed in BLOCK LETTERS using BLACK INK.<\/strong><\/p><ol><li><strong>Form <a href=\"https:\/\/dirco.gov.za\/uk\/wp-content\/uploads\/sites\/5\/2025\/10\/BI-85.pdf\">DHA\/BI-85 <\/a><\/strong><\/li><li><strong>DHA-24LRB:<\/strong> Must complete two forms (1 x to be completed in a new forename &amp; 1 x old forename as it is at present).<\/li><li><strong>DHA-24A<\/strong> to be completed for a child between 7 to 15 years of age.<\/li><li><strong>FORM DHA-9<\/strong> &#8212; To be completed for child who is 16 and above.<\/li><li><strong>FORM <a href=\"https:\/\/dirco.gov.za\/uk\/wp-content\/uploads\/sites\/5\/2024\/04\/DHA-154.pdf\">DHA\/BI 154<\/a> \u2013 To be completed with the new forename. <\/strong><\/li><li><a href=\"https:\/\/dirco.gov.za\/uk\/wp-content\/uploads\/sites\/5\/2025\/06\/DHA-529.pdf\"><strong>DHA\/BI\u00a0 529<\/strong><\/a> &#8211; A<strong>ll questions on both the sides of the forms must be completed. Please do not answer \u201cN\/A\u201d, it is not an answer to any of the questions. <\/strong><\/li><\/ol><ul><li><strong>Part A:<\/strong> <strong>Part A:<\/strong> questions 7(if born outside SA only) 9, 10 and 11 are most important for the applicant. <strong><u>B. <\/u><\/strong>No.9 dates when child came with the parents who came to live\/work in UK for the 1<sup>st<\/sup> time.<\/li><\/ul><p><strong><u>number 10(d) must also be answered, by circling a No or a Yes. Circle \u201cNO\u201d if you did not apply for Retention. Circle \u201cYes\u201d if you previously applied for a Retention and attach your 1<sup>st<\/sup> Retention copy.<\/u><\/strong><\/p><ul><li><strong>Part B:<\/strong> The relevant marital status can be written next to the words \u201cMarital Status\u201d on the form (e.g., Never married\/ Married\/ Divorced\/Widowed). The details of the applicant\u2019s current or former spouse must then be completed on the DHA-529 whether deceased or divorced or widowed.<\/li><li><strong>Part C and D: <\/strong>Must both be completed in full, with the details of the parents of the applicant including their ID numbers and details of their citizenship (if any).<\/li><li><strong>Part E<\/strong>: Must be signed in the presence of Home Affairs Official. Valid telephone\/mobile number, email address (in block letters) and physical address must be provided.<\/li><\/ul><ol start=\"7\"><li><strong>Form <a href=\"https:\/\/dirco.gov.za\/uk\/wp-content\/uploads\/sites\/5\/2024\/06\/DHA-SVP-109-NAMABIAN-QUESTIONER.pdf\">SVP-109E<\/a>: <\/strong>For South African citizens born in Namibia.<\/li><\/ol><p><strong><u>REQUIRED DOCUMENTS <\/u><\/strong><\/p><p><strong>You must come to your appointment <u>with the original and two copies<\/u> of each document. <\/strong><\/p><p><strong>All documents issued in foreign language MUST be officially translated into English language.<\/strong><\/p><p><strong>The documents to be submitted are of the <\/strong><span style=\"color: #ff0000;\"><strong>child and also of both the parents<\/strong><\/span><strong>. <\/strong><\/p><p><strong><u>Documents of the child<\/u><\/strong><strong>:<\/strong><\/p><ul><li>Current valid passport (for at least 4 years)<\/li><li><strong>Vault Copy of Unabridged Birth Certificate<\/strong> (with both the parents\u2019 details on it).<\/li><li>Signed statement by both the parents confirming why they both wish to change the forename of the child.<\/li><li>Evidence of usage of the forename to be registered. This may be in the form of a statutory declaration by a South African citizen who is prepared to state that this is the forename currently being used.<\/li><\/ul><ul><li>Six, clear professionally taken, color photos of the full face of applicant, showing only head and shoulders measuring 35mm x 45mm. Without glasses and clear forehead (not covered by hair).<\/li><li>If child has dual nationality, please provide proof as to how and when the foreign nationality was acquired\/granted, such Naturalization \/Registration Certificate or foreign birth certificate issued by the <strong>Embassy\/<\/strong> letter from the authorities stating the date you acquired the nationality. <strong><u>Please note<\/u><\/strong>, a foreign passport is not a proof of foreign citizenship\/nationality as it does not indicate when(date) and how you acquired the citizenship.<\/li><\/ul><ul><li>g.
