CHANGE OF FORENAMES FOR MAJOR<\/h2>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t

This amendment is in terms of Section 26(2) of the Births and Deaths Registration Act, 1992.<\/p>

Application for corrections<\/em>, re-registration of a birth<\/em>, insertion of the biological father’s particulars<\/em>, change of forenames<\/em>, change of the surname of a minor<\/em>, change of forenames Major, Change of Forenames minor, Change of Gender, Alteration of date of birth are different and the applicable guidelines should be consulted.<\/p>

To apply for the change of forenames you must comply with the following<\/strong>:<\/p>

Appointment Requirements:<\/strong><\/p>

  1. Ensure to come with BLACK INK pen <\/strong>\u00a0<\/strong><\/span>\u00a0<\/strong><\/li>
  2. Your current passport must be valid at least for 5 years before you submit your application for change of forenames. \u00a0<\/strong><\/li>
  3. All application forms must be fully completed but not signed, as they must be signed in the presence of a Home Affairs Official.<\/strong><\/li>
  4. Ensure to come with all originals and 2 x copies of supporting documents, <\/strong><\/span>the official will certify them.<\/strong><\/li>
  5. Printed proof of payment<\/strong><\/li><\/ol>
    1. A pre-paid self-addressed A4 envelope<\/span> (which indicates 500grm paid for).<\/li><\/ol>

      to post the outcome on receipt from the Department in South Africa.<\/p>

      1. You can only apply for the new passport in the new forenames once you receive your unabridged birth certificate with the new forenames<\/strong><\/li><\/ol>

        Prescribed forms and Documents \u00a0<\/strong><\/p>

        The Forms which cannot be downloaded will be given when you come to the office with the appointment.<\/strong><\/p>

        All forms must be fully completed in BLOCK LETTERS and BLACK INK <\/strong><\/p>

        1. Form DHA\/BI-85 <\/a><\/strong><\/li>
        2. DHA-24LRB<\/strong> — Must complete two similar forms. (1 x to be completed in a new surname & 1 x old surname as it is now). It will be given on the day of the appointment.<\/li>
        3. Form DHA \u2013 1688<\/strong> — It will be given on the day of the appointment!<\/li>
        4. FORM DHA-9<\/strong> — It will be given on the day of the appointment.<\/li>
        5. DHA\/BI 529<\/strong><\/a> Prescribed forms<\/strong>: DHA 529, to be completed in BLACK INK and BLOCK LETTERS in BLACK INK and BLOCK LETTERS (FULLY COMPLETED means all questions of the two forms must be answered. Please do not answer \u201cN\/A\u201d, it is not an answer to any of the questions. <\/strong><\/li><\/ol>
