It must have two unused pages. A certified true copy of the passport must accompany the application.<\/li><li>A fully completed application form (BI-84) signed by the applicant.<\/li><li>Prescribed fee of USD61, in case of paying applicants.<\/li><li>A valid yellow fever certificate and a certified true copy of the certificate.<\/li><li>Two colour passport photographs.<\/li><li>A return or onward ticket.<\/li><li>Proof of sufficient funds &#8211; to be proven by means of a 3 months original bank statement or certified copies thereof 9to include the current month), to cover air flight ticket and living expenses during the sojourn in the Republic.
