Must complete form\u00a0<\/strong>DHA-132<\/strong><\/a><\/p>\n\n

The Application must be submitted by post.<\/strong><\/p>\n\n

DEATH CERTIFICATE REQUIREMENTS:<\/strong><\/p>\n\n

Any person may apply for a copy of a Death Certificate provided that the deceased person\u2019s death has been registered with the South African Home Affairs.<\/strong><\/p>\n\n

If a foreigner died in South Africa, he\/she will be manually registered with South African Home Affairs and will be issued with handwritten death certificate. As they are handwritten certificates, is a time-consuming task with the result processing of applications are likely to take approximately 12 months or even longer.<\/strong><\/p>\n\n

Details on the application must be supplied according to the registration at death and not of names assumed in the family.<\/strong><\/p>\n\n

\u00a0REQUIREMENTS:<\/strong><\/p>\n\n

  1. Fully complete application form\u00a0DHA\/BI-132<\/a>\u00a0in black ink<\/li>\n\n
  2. Copy of previous death certificate (will facilitate the tracing and processing of the record)<\/li>\n\n
  3. Copy of ID document\/passport of the deceased<\/li>\n\n
  4. Processing fees (Non-Refundable) of Payment of\u00a0SEK57<\/strong>\u00a0to be deposited electronically into the Embassy Account<\/li>\n<\/ol>\n\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t
