Must complete form\u00a0<\/strong>DHA-154<\/strong><\/a>\u00a0in full.<\/strong><\/p>\n\n

Details on the application must be supplied according to the registration at birth and not of names assumed in the family. If the name\/forename change has taken place in South Africa through the Department of Home Affairs, a copy of approved letter or of a new amended certificate is required.<\/strong><\/p>\n\n

REQUIREMENTS:<\/strong><\/p>\n\n

Copy of any previous birth certificate (will facilitate the tracing and processing of the record).<\/p>\n\n

  1. Copy of ID document\/passport<\/li>\n\n
  2. A pre-paid A4 size self-addressed special deliver envelope.<\/li>\n\n
  3. Processing fees (Non-Refundable) of Payment of SEK 57<\/strong>\u00a0to be deposited electronically into the Embassy Account.<\/li>\n<\/ol>\n\n

    NP: Please be reminded that you will be required to submit printed proof of payment together with your application.<\/p>\n\n

    Please Note: To enable to facilitate the tracing of records and processing of the application, please submit fully completed applications including the Identity number or birth entry number.<\/p>\n\n
