REQUIREMENTS:<\/strong><\/p>\n\n

(This submission type is only for genuine emergencies such as death\/sickness in the family and is not in procession of a valid RSA passport)<\/p>\n\n

Citizens 16 years and above that have never been issued with an ID document\/smart card are not eligible for emergency travel documents until and unless they apply for an ID document at least three months before we can issue the travel document. \u00a0(and are still in possession of a child passport)<\/p>\n\n

Citizens under 16 years that do not have a 13-digit ID number are not eligible, until the birth is registered with Home Affairs and an ID number is allocated to them.<\/p>\n\n

Prescribed forms to be completed in BLACK INK and BLOCK letters:<\/strong><\/p>\n\n

  2. DHA 529 (BI-529)<\/a><\/li>\n\n
  3. DHA 9 (will be given at your appointment)<\/li>\n\n
  4. Police<\/a> report\u00a0(if the passport is lost\/stolen)<\/li>\n<\/ol>\n\n

    Supporting Documents\/Information<\/strong>:<\/p>\n\n

    1. RSA passport<\/li>\n\n
    2. ID book or Smart ID Card<\/li>\n\n
    3. Four photos<\/li>\n\n
    4. Proof of emergency (A signed doctor\u2019s letter on Doctor\u2019s letter head) \/RSA death certificate\/<\/li>\n\n
    5. Motivation letter, with SA address you are going to and stating the emergency, why you are not in possession of a valid passport, and you understand the emergency document is a one-way\/single-entry document.<\/li>\n\n
    6. If you have dual nationality, please provide proof as to how and when you acquired the foreign nationality, such Naturalization \/Registration Certificate or foreign birth certificate issued by the Embassy\/ letter from the authorities stating the date you acquired the nationality. (Please note foreign passport is not a proof of foreign citizenship\/nationality as it does not indicate as to when(date) and how you acquired the citizenship)\u00a0\u00a0<\/li>\n\n
    7. If the document is in a foreign language, then it must be a sworn translation by an official translator in English language.<\/li>\n\n
    8. Letter of retention of your South African Citizenship.<\/li>\n\n
    9. Certified copies of both the parents\u2019 ID documents and passports if you are under 18 years.<\/li>\n\n
    10. If under the age of 18years, proof of both the parents\u2019 foreign nationality such as naturalization\/Registration certificate or foreign birth certificate issued by the Embassy\/ letter from the authorities stating the date you acquired the nationality. (Please note foreign passport is not a proof of foreign citizenship\/nationality as it does not indicate as to when(date) and how you acquired the citizenship)<\/li>\n\n
    11. Processing fees (Non-Refundable) for an adult ETC is SEK 104 which must be deposited electronically into the Embassy Account.<\/li>\n<\/ol>\n\n
