Overview:<\/strong><\/p>\n\n

To register the birth of a child and apply for their first South African passport, the following steps and requirements must be adhered to:<\/p>\n\n

Appointment Requirements:<\/strong><\/p>\n\n

    \n
  1. Both parents must be present at the appointment to register the birth.<\/li>\n\n
  2. Children aged seven and above must also attend the appointment.<\/li>\n\n
  3. All application forms must be fully completed but not signed, as they must be signed in the presence of a Home Affairs Official.<\/li>\n\n
  4. Ensure all original documents and copies are brought to the appointment; the official will certify the copies.<\/li>\n<\/ol>\n\n

    Forms and Documentation:<\/strong><\/p>\n\n

      \n
    1. Some forms are available for download; others will be provided at the office on the day of the appointment.<\/li>\n<\/ol>\n\n

      To apply for birth registration and a first passport, comply with the following:<\/p>\n\n

        \n
      1. Forms to be Completed:<\/strong><\/li>\n<\/ol>\n\n

        \u00a0 \u00a0 \u00a0 \u00a0 \u00a0Prescribed forms to be completed in\u00a0BLACK INK and BLOCK LETTERS<\/strong><\/p>\n\n

          \n
        1. Form DHA-24<\/strong>\u00a0for child to be registered\u00a0before 30 days<\/strong>. (with four photos)<\/li>\n\n
        2. Form DHA-24 and\u00a0<\/strong>DHA-288A<\/strong><\/a>\u00a0for child to be registered\u00a0before 12 months<\/strong>. (with four photos)<\/li>\n\n
        3. Form DHA-24LR and DHA-288<\/strong>\u00a0for registration of children aged between\u00a01 YEAR to 6 years<\/strong>. (with 6 photos)<\/li>\n\n
        4. Form DHA-24LR, DHA-288 and DHA-24A<\/strong>\u00a0for registration of aged between\u00a07 to 15 years<\/strong>. (with 6 photos)<\/li>\n\n
        5. Form DHA-24LR, DHA-288, DHA-24A and 2x DHA-9<\/strong>\u00a0for registration of children aged between\u00a015 onwards<\/strong>\u00a0(with 8 photos).<\/li>\n\n
        6. Form\u00a0<\/strong>DHA- 288C<\/strong><\/a>,\u00a0completed by the father<\/strong>\u00a0to be completed by the father if the child is born out of wedlock (do not sign the form).<\/li>\n\n
        7. Form:\u00a0<\/strong>DHA-73<\/strong><\/a>, The form is available for download; please fill it out completely but refrain from signing it.<\/li>\n\n
        8. Form:\u00a0<\/strong>DHA-529<\/strong><\/a>\u00a0x 3<\/strong>, fully completed on both sides of the form (do not write N\/A and do not sign). Complete one form in the child\u2019s name and one form for each SA parent.<\/li>\n<\/ol>\n\n

          The purpose of the form is to establish that you have not lost your Citizenship and that all your details mentioned therein are according to the National population registrar.\u00a0<\/p>\n\n

          2. Specific Sections to Complete:<\/strong><\/p>\n\n

            \n
          1. Part A: Questions 7 (if born outside SA), 9, 10, and 11 are critical.<\/li>\n\n
          2. Part B: Indicate marital status and complete details of the current or former spouse.<\/li>\n\n
          3. Part C and D: Both parents\u2019 details, including ID numbers and foreign nationality, must be completed.<\/li>\n\n
          4. Part E: Ensure the DHA-529 form is signed in the presence of a Home Affairs Official and provide valid contact details.<\/li>\n<\/ol>\n\n

            Part A:<\/strong>\u00a0Questions 7 (applicable only if born outside South Africa), 9, 10, and 11 are the most important for the applicant.\u00a0N.B<\/strong>. For item 9, enter the dates when you first came to live or work in Sweden\/Finland\/Lativa\/Lithuania and Estonia. For item 10(d), you must also respond by circling either \u201cNo\u201d or \u201cYes.\u201d Circle \u201cNo\u201d if you have never applied for the Retention of South African citizenship letter; circle \u201cYes\u201d if you have previously applied for a Retention letter. Remember to bring your Retention letter or certificate with you to your appointment.<\/p>\n\n

            Part B:<\/strong>\u00a0Next to the \u201cMarital Status\u201d section on the form, the appropriate status should be indicated (e.g., Single, Married, Divorced, Widowed). Subsequently, the details of the applicant\u2019s current or former spouse should be provided on the form\u00a0DHA-529<\/a>, whether deceased or divorced or widowed)<\/p>\n\n

            Part C and D<\/strong>: Both the parents\u2019 details must be completed including their ID numbers and foreign nationality (if any).<\/p>\n\n

            Part E<\/strong>: Ensure that form\u00a0DHA-529<\/a>\u00a0is signed in the presence of Home Affairs Official. Valid telephone\/mobile number, email address (in block letters) and physical address must be provided.<\/p>\n\n

            DOCUMENTS TO BRING WITH YOU ON THE DAY OF YOUR APPOINTMENT: (<\/strong>Original plus two copies):<\/p>\n\n

              \n
            1. Child\u2019s (applicant\u2019s) unabridged birth certificate (where both the parents\u2019 names are mentioned)<\/li>\n\n
            2. Proof of foreign nationality of the child.<\/li>\n\n
            3. All above mentioned photos must be very clear, professionally taken colour photos of the full face of applicant, showing only head and shoulders measuring 35mm x 45mm, with a clear forehead (Not covered by hair)<\/li>\n\n
            4. Please Note:<\/strong>\u00a0Birth Registration and the 1st<\/sup>\u00a0birth certificate will be issued free of charge.<\/strong><\/li>\n\n
            5. Processing fees (Non-refundable<\/strong>) It must be paid in bank and in Swedish Krone of\u00a0732\u00a0<\/strong>for the new passport, it must be deposited electronically into the bank account<\/li>\n\n
            6. NB: Please be reminded that you will be required to present printed proof of payment when you attend the appointment.<\/li>\n<\/ol>\n\n

              PARENTS\u2019 DOCUMENTS REQUIRED: (original plus 2 x copies)<\/strong><\/p>\n\n

                \n
              1. Marriage certificate. If married outside South Africa<\/li>\n\n
              2. Both the parents\u2019 documents ID documents, passports, SA permits, SA birth certificates. If they have dual nationality, please provide proof as to how and when you acquired the foreign nationality, such Naturalization \/Registration Certificate or foreign birth certificate issued by the Embassy\/ letter from the authorities stating the date you acquired the nationality. (Please note a foreign passport is not a proof of foreign citizenship\/nationality as it does not indicate as to when(date) and how you acquired the citizenship).<\/li>\n\n
              3. \u00a0If you have any other nationality and you do not have any of the above\u00a0documents,\u00a0then please contact the country\u2019s authorities and request a letter\u00a0stating the\u00a0date and means<\/strong>\u00a0of you acquiring the citizenship\/nationality.<\/li>\n\n
              4. If any document is in a foreign language, you must get them officially translated in English language. You must bring in copies of both the documents. i.e. document in foreign language and also officially translated copies in English.<\/li>\n\n
              5. Retention of SA citizenship letter (if you have one)<\/li>\n<\/ol>\n\n

                Originals of\u00a0all\u00a0the documents are needed.<\/strong><\/p>\n\n
