UNABRIDGED DEATH CERTIFICATE APPLICATION
Must complete form DHA-132
The Application must be submitted by post.
DEATH CERTIFICATE REQUIREMENTS:
Any person may apply for a copy of a Death Certificate provided that the deceased person’s death has been registered with the South African Home Affairs.
If a foreigner died in South Africa, he/she will be manually registered with South African Home Affairs and will be issued with handwritten death certificate. As they are handwritten certificates, is a time-consuming task with the result processing of applications are likely to take approximately 12 months or even longer.
Details on the application must be supplied according to the registration at death and not of names assumed in the family.
REQUIREMENTS:
- Fully complete application form DHA/BI-132 in black ink
- Copy of previous death certificate (will facilitate the tracing and processing of the record)
- Copy of ID document/passport of the deceased
- Processing fees (Non-Refundable) of Payment of SEK57 to be deposited electronically into the Embassy Account
