Details on the application must be supplied according to the registration at birth and not of names assumed in the family. If the name/forename change has taken place in South Africa through the Department of Home Affairs, a copy of approved letter or of a new amended certificate is required.

REQUIREMENTS:

Copy of any previous birth certificate (will facilitate the tracing and processing of the record).

1. Copy of ID document/passport
2. A pre-paid A4 size self-addressed special deliver envelope.
3. Processing fees (Non-Refundable) of Payment of SEK 57 to be deposited electronically into the Embassy Account.

NP: Please be reminded that you will be required to submit printed proof of payment together with your application.

Please Note: To enable to facilitate the tracing of records and processing of the application, please submit fully completed applications including the Identity number or birth entry number.

The tracing of original records in the register held by the Department of Home Affairs in Pretoria is a time-consuming task, as a  result processing of applications is likely to take approximately 12 months or longer.
