1. Fully complete application form DHA/BI-154 in black ink

2. Copy of an old birth certificate

3. Copy of ID document/passport

4. Payment of £7.00 to be deposited electronically into the High Commissioner’s Account (see

below information)

5. A pre-paid (100 grams stamp) A4 size self-addressed special deliver envelope.

MARRIAGE CERTIFICATE REQUIREMENTS:

1. Fully complete application form DHA/BI-130 in black ink

2. Copy of an old marriage certificate

3. Copy of ID document/passport of both the parties

4. Payment of £7.00 to be deposited electronically into the High Commissioner’s Account (see

below information)

5. A pre-paid (100 grams stamp) A4 size self-addressed special deliver envelope.

DEATH CERTIFICATE REQUIREMENTS:

1. Fully complete application form DHA/BI-132 in black ink

2. Copy of an old death certificate

3. Copy of ID document/passport

4. Payment of £7.00 to be deposited electronically into the High Commissioner’s Account (see
