Application For Amendment of Personal Particulars<\/strong><\/p>

These guidelines are for amendments in terms of Section 7(2) of the Births and Deaths Registration Act, 1992.<\/p>

This application is to\u00a0correct an incorrect\u00a0surname, name, date of birth, place of birth or gender, as it appears in the National Population Register.<\/p>

Application for\u00a0re-registration of a birth,\u00a0insertion of the biological father’s particulars,\u00a0change of the surname of a major,\u00a0change of the surname of a minor, change of forename(s) of a minor or major, is different and the applicable guidelines should be consulted.<\/p>

Section 17 of The Children’s Act (Act No. 38 of 2005) stipulates that a child becomes a major upon reaching the age of 18 years.<\/p>

The following documentation is required:<\/p>

  1. Completion of forms BI-526<\/a>, BI-24<\/a>, BI-529<\/a><\/li>
  2. Form DHA-9 – All applications for amendments of persons 16 years and older\u00a0MUST<\/strong>\u00a0include the finger print form. To obtain the DHA-9 finger print form\u00a0please send a self-addressed prepaid ordinary or express post envelope to the South African High Commission, State Circle, Yarralumla, Canberra 2600.\u00a0Please include a cover note requesting the form and quoting applicant(s) full name and RSA ID number(s).\u00a0(If more than one DHA-9 form is required please send a separate envelope for each form or one large envelope to accommodate all the forms)<\/strong>.<\/li>
  3. The request for any correction must be supported by the necessary documentary proof.<\/li>
  4. To support and assist with the finalization of the application, copies of the ID, Passport, Marriage Certificate or Birth Certificate of the parent\/s should be submitted.<\/li><\/ol>

    Application fee<\/strong>:<\/p>

    Non-refundable fee of\u00a0AUD 6.00<\/strong>\u00a0per application.<\/p>

    In terms of section 27(A) read with the provisions of the\u00a0Alteration of Sex Description and Sex Status Act<\/strong>, 2003 (Act No.49 of 2003).<\/p>

    Applications can be made by:<\/p>

    Persons who have undergone a sex change operation or medical treatment resulting in their gender reassignment.<\/strong>\u00a0In such cases two medical reports are required to establish his \/ her gender:<\/p>

    1. One by the medical practitioner who applied the procedure or medical treatment or by a medical practitioner who has experience in such procedures or treatments, and;<\/li>
    2. A report by a second medical practitioner who has independently examined the applicant.<\/li><\/ol>

      Inter-sexed persons.<\/strong>\u00a0In this category, the applicant must submit:<\/p>

      1. A report by a medical practitioner corroborating that the applicant is inter-sexed, as well as;<\/li>
      2. A report by qualified psychologist or social worker corroborating that the applicant is living and has lived stably and satisfactorily, for an unbroken period of at least two years in the gender role corresponding to the sex description under which he or she seeks to be registered.<\/li><\/ol>

        Payment must be in Australian Dollars (AUD) made out to the “South African High Commission” by way of Postal Money Order or Bank Cheque.<\/p>

        Personal cheques or credit cards are NOT accepted. Incorrect payment will not be accepted; in case of incorrect payment the cheque or postal money order will be returned to applicant.<\/p>
